بررسی و تحلیل عدالت در سلامت شهری محدودۀ. مورد مطالعه: شهر بجنورد

چکیده

سلامت شهری تنها به معنای نبود بیماری در شهر نیست، بلکه به معنای تندرستی کامل جسمی، روانی و اجتماعی همۀ ساکنان شهر و همچنین محیط فیزیکی و فضای اجتماعی، فرهنگی، اقتصادی و سیاسی سالم در شهر است. هدف اصلی برنامه‌ریزی سلامت شهری دستیابی عادلانه همۀ ساکنان به سلامت کامل جسمی، روانی و اجتماعی و همچنین دستیابی عادلانه به عوامل تعیین‌کنندۀ‌ سلامت شهری می‌باشد. امروزه موضوع عدالت در سلامت شهری، به یکی از مهم‎ترین و با اولویت‌ترین موضوع‌‌های جهان و کشورها تبدیل شده است. عدالت در سلامت شهری به معنای نبود تفاوت‌‌های سیستماتیک در یک یا چند جنبه از وضعیت سلامت شهروندان یا گروههای جمعیتی از نظر اجتماعی، اقتصادی و محیط فیزیکی است. با توجه به اهمیت موضوع عدالت در سلامت شهری، در این پژوهش به بررسی عدالت در سلامت در شهر بجنورد پرداخته می‌شود. روش تحقیق در این پژوهش از نظر هدف، کاربردی، از نظر روش انجام تحقیق، توصیفی-تحلیلی، از نظر گردآوری اطلاعات، کتابخانه‌ای و پیمایشی و از نظر تجزیه‌و‌تحلیل اطلاعات، کمّی می‌باشد. نتایج شاخص‌های عدالت در سلامت شهری در شهر بجنورد نشان داد که ساکنان شهر بجنورد به هیچ­یک از عوامل تعیین‌کننده سلامت شهری (عوامل اقتصادی، اجتماعی، فیزیکی-کالبدی و خدمات بهداشتی-درمانی) دسترسی عادلانه ندارند. طبق نتایج، با فاصله گرفتن از مرکز شهر، میزان دستیابی به تعیین‌کننده‌های سلامت شهری دشوارتر می‌شود؛ بنابراین، حاشیه‌نشینان شهر بجنورد کمترین میزان دسترسی به عوامل تعیین‌کننده سلامت را دارند. از نظر سرانه خدمات بهداشتی-درمانی محله 12 و 2-9 با چیرگی 9 و محله 4-7 با چیرگی 38، از نظر شاخص‌های اجتماعی محله 12 با میانگین 44 و محله 3-8 با میانگین 61، از نظر قابلیت پیاده‌روی محله 12 با میانگین 35 و محله 3-8 با میانگین 42، به ترتیب دارای کمترین و بیشترین سطح دسترسی به عوامل تعیین‌کننده سلامت می‌باشند. به‌طورکلی می‌توان گفت، سلامت شهری یک مفهوم پیچیده است که عوامل گوناگون مانند عوامل اجتماعی-فرهنگی، اقتصادی، محیط فیزیکی، نظام سلامت شهری و ساختار سیاسی بر آن تأثیر می‌گذارند؛ بنابراین تأمین سلامت شهروندان موضوعی نیست که بتوان آن را از طریق برنامه‌ریزی و اقدامات بخشی به­دست آورد. تأمین سلامت شهروندان، نیازمند برنامه‌ریزی بلندمدت، بین‌بخشی و یکپارچه است که همۀ افراد و سازمان‌ها باید برای دستیابی به آن تلاش کنند.

چکیده [English]

Urban health is not merely the absence of illness in a given city. Rather, it is the complete physical, mental and social well-being of all the citizens as well as a healthy physical, social, cultural, economic and political urban environment. The main purpose of urban health planning is to achieve equitable access for all citizens to full physical, mental and social wellbeing and to the determinants of urban health. Today, the issue of urban health equity has become one of the most pressing issues in different countries around world. Urban health equity means the absence of systematic differences in one or more aspects of the health status of people or of demographic groups in terms of socio-economic factors and the physical environment. Given the importance of the issue, this study made an attempt to investigate urban health equity in Bojnourd city. The present study uses a quantitative, exploratory, applied-developmental, descriptive-analytical design in terms of data analysis, data collection, objectives, methods and procedures, respectively. According to the results, the residents of Bojnourd do not have equitable access to any of the factors that define urban health, including economic, social, physical-spatial and health care related factors. Access to the determinants of health equity becomes more and more difficult as we move away from downtown Bojnourd. As a result, people living in the suburbs of Bojnourd have the least access to health determinants. In general, urban health is a complex concept that is affected by various factors such as social, cultural and economic factors; the physical environment; the urban health system; and, the political structure. Therefore, citizen health is not something that can be achieved through sectoral planning or measures. Citizen health requires long-term, inter-sectoral and integrated planning with the involvement of all individuals and organizations.
